聚乙二醇电解质散联合西甲硅油在胶囊内镜检查中的应用

【摘 要】 目的 观察聚乙二醇电解质散联合不同剂量西甲硅油在胶囊内镜检查中应用效果。方法 将80 例行胶囊内镜检查的健康体检者随机分为实验组（聚乙二醇电解质散2 盒、西甲硅油300 mg）和对照组（聚乙二醇电解质散2 盒、西甲硅油80 mg），观察患者肠道清洁程度以及肠道内气泡存在情况.并观察有无不良反应。结果 两组肠道清洁有效率均达100%，无统计学差异；实验组小肠肠道内的气泡量少于对照组，二组比较差异有统计学意义(p＜0.05)。均无出现明显不良反应。结论 聚乙二醇电解质散联合西甲硅油应用于胶囊内镜检查，能够有效清洁肠道、祛除肠道内的气泡，其祛泡效果还与西甲硅油的使用剂量相关。 【关键词】 聚乙二醇电解质散；西甲硅油；胶囊内镜 [Abstract] Objective To observe the effectiveness of polyethylene glycol electrolyte powder plus different dose of simethicone in the Capsule endoscopy examination. Methods 80 health people were randomly divided into experimental group and control group. The experimental group was given two boxes of polyethylene glycol electrolyte powder plus 300 mg of simethicone, and the control group was given two boxes of polyethylene glycol electrolyte powder plus 80 mg of simethicone, then observe the intestinal tract of cleanliness level and the presence of air bubbles, and observe if some adverse reactions were happened. Results Both groups of the efficiency rate of cleanliness level were 100%, no significant difference between them. To observe The bubbles of the bowel, the experimental group’s was less than the control group’s, there was statistically significant difference between two groups (P 0.05). And there was no apparent adverse reactions in two groups. Conclusion polyethylene glycol electrolyte powder plus simethicone was used in Capsule endoscopy examination could effectively clean the intestinal tract and obviously eliminate the bubbles of the bowel, and the cleanliness level was relevant to the dose of simethicone.

[Keywords] polyethylene glycol electrolyte powder; simethicone; Capsule endoscopy 胶囊内镜是小肠疾病最重要的检查手段之一，由于胶囊内镜不具备传统胃肠镜的冲洗、抽吸功能，因此，良好的肠道准备在胶囊内镜检查中至关重要。如何提高胶囊内镜检查中肠道准备的质量，国内外学者做了大量研究工作，还未取得一致标准。我们采用聚乙二醇电解质散联合西甲硅油进行肠道准备，收到满意的效果，现报告如下。 1 资料与方法 1.1 资料来源 自2008 年1 月至2011 年6 月到我院行胶囊内镜检查的健康体检者，随机分为实验组（聚乙二醇电解质散2 盒、西甲硅油300 mg）和对照组（聚乙二醇电解质散2 盒、西甲硅油80 mg）各40 例，两组在年龄、性别差异上无统计学意义，具有可比性。 1.2 试剂及仪器 西甲硅油乳剂（柏西，艾普米森，柏林—化学股份公司）；OMOM胶囊内镜（重庆金山科技）；聚乙二醇电解质散（恒康正清 江西恒康药业有限公司）。 1.3 实施方法 检查前1天进流质饮食，晚上7 时开始服用聚乙二醇电解质溶液（配制方法：将聚乙二醇电解质散每1盒内的3 小袋药粉完全溶于1000 ml温开水中，即可服用。用量为2 盒，配制成2000 ml，首次口服500 ml，剩余的1500 ml在2 小时内服完），直至排出液清亮为止，然后禁食12 小时以上。 吞服智能胶囊前半小时口服西甲硅油：实验组300 mg、对照组80 mg。吞服智能胶囊后2 小时内禁食，2 小时后可饮少量清水，4 小时后可进食少许干粮，检查结束（约8 小时）后方可恢复正常饮食。 1.4 观察指标 由专职内镜医师进行阅片，阅片医师对分组情况并不知晓。阅片医师根据小肠肠道内的清洁程度、气泡量各分为3 个等级： ①清洁程度：优：肠道内清洁，无粪水，不影响视野；良：肠腔内有少量粪渣及粪水，但不影响视野；差：肠腔内粪渣及粪水较多，影响视野。优和良视为有效等级，差视为无效等级。 ②气泡量：A级：无气泡或少量气泡，对粘膜观察基本无影响；B级：中量气泡，对粘膜观察有轻度影响；C级：大量气泡，对粘膜观察有较大影响。A级和B级视为有效等级，C级视为无效等级。
