皮管快速扩张的实验研究和临床应用
【摘要】 目的 研究动物皮管快速扩张的效果，为临床应用提供依据。方法 取健康家兔12只，雌雄不限，体重1.5～2.0kg。在背部脊柱两侧各设计二对皮管，快速扩张组每天扩张囊注水1次，注水量为扩张囊容量的10%～15%，常规扩张组每周扩张囊注水2次，注水量为扩张囊容量的10%～15%。临床应用:6例病人中，2例为膝关节瘢痕挛缩，4例为会阴瘢痕挛缩;每天扩张囊注水1次，注水量为扩张囊容量的10%～15%。结果 动物实验:快速扩张组24个皮管，16个皮管完成扩张，扩张时间7天，8个皮管因感染、扩张器漏水等原因失败;常规扩张组24个扩张器18个完成全部扩张，4个皮管因感染、1个皮管因扩张器注水困难、1个皮管因扩张器外露未完成扩张。临床应用:6例病人6个皮管完成扩张，注水时间7天，皮瓣转位修复创面，全部成活。结论 皮管快速扩张是安全可行的。
【Abstract】 Objective To study the effect of the rapid expanded skin tube，provide evidence for clinical apˉplication.Methods Design2skin tubes in each side of the backbone of health rabbits，total12rabbits in spite of sex.Weight range from1.5to2.0kg.One side is the rapid expanded skin tube group，injecting10%—15%，volume water into the expanded tube once a day.The other side is routine expended skin tube group，injecting10%—15%volume water into the expanded tube twice everyweek.Clinical application:In6patients，2cases are contracture of the scar ofknee joint;the other4case are contracture of the scar of perineum.Each of them inject10%—15%volume water in the expander.Results Animal experiment:In rapid expanded skin tube group of24cases.16cases had finˉished the experiment for7days.the failure of the other8cases due to infection and leakage of the expander.In routine expanded skin tube group of24case.18cases ahd finished the experiment，and 4 cases failure due to infection，1case due to difficult injecting，1case due to exposure of expander.Clinical application:all of6cases had finished the experˉiment.for7days all the skin flap survived completely offer repaired the tissue defect adjacently.Conclusion The rapid expanded skin tube can be clinical aplicated safely.

1976年皮肤软组织扩张器开始应用于临床，解决了较大皮肤组织缺损修复的难题，而且扩张后的皮肤在色泽、弹性、厚薄等都与所修复的组织相近，但常规使用皮肤软组织扩张器每周注水扩张1～2次，完成注水需要3～4周时间，注水扩张周期长;为缩短注水扩张周期，我科于1999年开始进行皮管快速扩张的动物实验和临床研究，取得满意效果，报告如下。
1 实验资料。
1.1 实验方法 取健康家兔12只，雌雄不限，体重1.5～2.0kg（中山大学动物实验室）。麻醉采用氯胺酮静脉麻醉，用量10mg/kg;术区剃毛，用2%碘酒、75%酒精消毒，铺无菌手术巾。每只家兔在脊柱两侧各设计两个皮管，长3.5cm，宽1.5cm，切开皮肤至肉膜层，在肉膜层深面分离皮瓣制成皮管，供瓣区直接间断缝合，皮管内放置10ml微型扩张器，注射壶内置（扩张器由华南理工大学钰霖生物材料有限公 司生产，批号990130），10天拆线后开始注水扩张。每只家兔脊柱两侧左上右下扩张器为常规扩张组，左下右上扩张器为快速扩张组，扩张液用0.9%NaC1注射液，用5号注射针头注水扩张，常规扩张组每3天注水1次，每次注水1～1.5ml，3～4周完成扩张;快速扩张组每天注水1次，每次注水1～1.5ml，7～10天完成扩张。每次注水后扩张后皮瓣颜色变化情况，分别测量皮管的皮温（采用上海医用仪表厂生产的WMY—01数字温度计）、囊内压力。
1.2 实验结果。
1.2.1 一般观察 每次注水皮瓣颜色由红润转变为苍白，大约1～3min逐渐转为红色;如果3～5min后仍然是苍白，则将囊内液放出少许至颜色变成红色为止。
1.2.2 囊内压测定 扩张后立即测量囊内压，常规扩张组为（64.5±12）mmHg，快速扩张组为（71.25±10.5）mmHg;术后24h测量囊内压，常规扩张组为（56.25±11.25）mmHg，快速扩张组为（58.5±9）mmHg。
1.2.3 扩张后皮温 常规扩张组为（35±0.8）℃，快速扩张组为（32±0.5）℃;术后24h测量，常规扩张组为（37±0.5）℃，快速扩张组为（37±0.6）℃。
