局部注射丝裂霉素治疗翼状胬肉的疗效观察

【摘要】 目的 探讨局部注射丝裂霉素治疗早期翼状胬肉的疗效。方法 选择胬肉组织侵入角膜内2 mm的患者386例464眼，在显微镜下，将0.2 mg/ml的丝裂霉素溶液0.1～0.3 ml注入胬肉头颈体部。术后滴复方妥布霉素眼液，涂复方妥布霉素眼膏，常规疗程1周。1周后同法治疗。观察角膜浸润、胬肉血管及胬肉组织的肥厚程度。随访不少于6个月。结果 治愈436眼，好转28眼，无效0眼。出现点状角膜炎6例6眼，经治疗均痊愈。结论 局部注射丝裂霉素能有效治疗早期翼状胬肉。
［Abstract］ Objective To evaluate the efficacy therapeutic effect of regional injection of mitomycin C for treating patients with pterygium.Methods 464 eyes of 386 patients with corneal incursion less than 2 mm were received 0.1～0.3 ml of 0.2 mg/ml mitomycin C locally injected into the head and body of pterygium under microscope.Subsequently，all eyes were treated with tobramycin and dexamethasone ophthalmic suspension （qid） and tobramycin and dexamethasone ophthalmic ointment （qn） for 1 week.And then，the same therapy above—mentioned were repeated again.The corneal infiltration，blood vessels and plump degree of pterygium were observed.The follow—up of 6 months and over were carried out to all of patients.Results After local injection of mitomycin C，the pterygium were resolved in 436 eyes，and improved in 28 eyes.6 eyes of 6 patients were suffered from punctuate keratitis and healed within one month.Conclusion Localized mitomycin C is an effective treatment for pterygium in early phase.

翼状胬肉（以下简称胬肉）是肥厚的结膜和结膜下组织由球结膜向角膜侵袭，是一种常见多发病，胬肉不但给患者带来容貌方面的困惑，同时，病程漫长，局部经常充血发炎，给患者带来许多的不适症状，影响患者的生存质量。胬肉治疗方法较多，目前仍以手术为主，但术后复发率高达20%～50%，因而仍是眼科医师较棘手的问题之一。我们在门诊对早期的胬肉进行局部注射丝裂霉素（0.2 mg/ml）治疗，收到良效，现报告如下。
1 资料与方法。
1.1 一般资料 自2000年1月至2006年6月，我院门诊早期翼状胬肉患者（胬肉组织侵入角膜内小于2 mm）386例464眼，男168例184眼，女218例280眼；年龄最大55岁，最小25岁，平均（38.5±0.21）岁。
1.2 方法 日本株式会社产丝裂霉素注射粉剂2 mg+无菌生理盐水5 ml＋2%利多卡因5 ml，置于4 ℃冰箱保存（最长时间不超过1周）备用。患眼用0.5%地卡因表面麻醉后，用1 ml注射器抽取0.5 ml备用的丝裂霉素溶液（0.2 mg/ml），在手术显微镜下，针头从胬肉颈部上方进针，将0.2 mg/ml的丝裂霉素溶液0.1～0.3 ml注入胬肉头颈体部。1周后，同法进行局部治疗。术后涂复方妥布霉素眼膏，复方妥布霉素滴眼液点术眼，qid，复方妥布霉素眼膏涂术眼，qn，持续用药1周。出现角膜上皮点状着色者，给予重组人表皮细胞生长因子衍生物滴眼液，重组牛碱性成纤维细胞生长因子眼用凝胶，qn，口服消炎痛25 mg，维生素C片0.2 g，维生素B1片20 mg，tid。
1.3 疗效判定标准 术后观察至少6个月。治愈：角膜无浸润灶，胬肉头苍白萎缩、无血管侵入，胬肉体变薄、血管减少；好转：角膜无浸润灶，胬肉头萎缩、胬肉头体部血管减少；无效：各项指标与治疗前相同。
