两种不同的抗凝剂全血在血沉检测中的应用

【摘要】 目的 对枸橼酸钠盐抗凝全血与乙二胺四乙酸盐(EDTA—K2)抗凝全血进行血沉检测比较,探讨EDTA—K2抗凝全血用于血沉检测的可行性。方法 选用109mmol/L枸橼酸钠盐抗凝全血与EDTA—K2抗凝全血,同时用传统的魏氏法血沉自动仪进行检测。结果 枸橼酸钠抗凝魏氏法组与枸橼酸钠抗凝仪器法组比较,P0.05,枸橼酸魏氏法组与EDTA—K2魏氏法组比较P0.05,枸橼酸钠抗凝仪器法与 EDTA—K2仪器法组比较P0.05,EDTA—K2魏氏法与 EDTA—K2仪器法组比较P0.05。四个测定组间血沉值的比较差异均无显著性。结论 EDTA—K2抗凝全血可以用于血沉检测

【关键词】 血沉枸橼酸钠;乙二胺四乙酸盐;魏氏法血沉仪   【Abstract】 Objective To explore the feasibility of whole blood sample anticoagulated by EDTA—K2 in ESR examination.Methods Whole blood samples anticoagulated respectively by sodium citrate and EDTA—K2 were collected.The erythrocyto sedimentation rate(ESR) of all samples were detected by traditional manual westergren method and automated system.Results There was no significant difference between manual westergren method and automated system in ESR detection,no matter the sample anticoagulated by sodium citrate or EDTA—K2(P0.05).The ESR of whole blood anticoagulated by sodium citrate was similar with that by EDTA—K2,which had nothing to do with method (P0.05).Conclusion The whole blood anticoagulated by EDTA—K2 can be used for ESR examination.

血沉检测过去选用109mmol/L枸橼酸钠盐作为抗凝剂。自从血细胞仪普及,血常规采血由末梢血改为静脉血以来,对于用乙二胺四乙酸盐(EDTA—K2)抗凝全血代替枸橼酸钠盐抗凝全血用于血沉检测,文献中已有报道,其中方法多数是采用EDTA—K2抗凝血中加入枸橼酸钠或者生理盐水进行稀释,采用全血进行血沉检测较少报道。笔者用EDTA—K2抗凝全血与枸橼酸抗凝全血,同时用魏氏法仪器法进行血沉检测,现报告如下。

1 资料与方法。

1.1 一般资料。

选用我院住院患者34例,男22例,女12例,年龄25~80岁。红细胞压积0.3~0.35。

1.2 方法[1,2]。

(1)静脉采血1.6ml注入含有枸橼酸钠0.4ml的抗凝管中,混匀。(2)选用同一患者做血常规后EDTA—K2抗凝血2ml(血常规用量0.1ml)。(3)枸橼酸抗凝血全血用传统的魏氏法血沉仪器法进行检测,EDTA—K2抗凝全血同样用传统的魏氏法血沉仪器法进行检测

1.3 仪器

(1)魏氏法魏氏管和血沉架,按操作规程操作。(2)由DRAGONED提供2010血沉仪。
