纵隔气肿的X线和螺旋CT诊断价值

【摘要】; 目的：探讨X线胸片和螺旋CT对纵隔气肿的诊断价值。方法：回顾性分析50例纵隔气肿的X线胸片（36例）和螺旋CT（41例）表现。结果：50例纵隔气肿病例，X线胸片显示纵隔气肿呈现透亮影像，侧位片比正位片显示更清楚，螺旋CT可清晰显示纵隔气肿呈现气体密度，以及被气体勾画出的纵隔结构。结论：X线胸片和螺旋CT检查有利于早期诊断纵隔气肿，对临床及时诊断和治疗具有重要的应用价值。 毕业论文。

; Abstract: Objective To study the clinical value of chest X?ray and spiral CT scan in diagnosis of mediastinal emphysema.Methods Review analysis 50 cases with mediastinal emphysema, and which were had the integrity chest X?ray and helical CT material. Results In 50 cases with mediastinal emphysema,The chest X?ray demonstrates the translucent image, and side position was clearer than posteri?anterior. The spiral CT clear display mediastinal emphysema presents the density of gas,and mediastinal structure was outlined which by the gas. Conclusion The chest X?ray and spiral CT were advantageous to the early diagnosis mediastinal emphysema and it had the important value to the clinical prompt diagnosis and treatment. 毕业论文。

; 纵隔气肿是胸部结构内病理性气体的异常积聚。临床上较为少见，加之部分患者无典型的临床症状，因此放射学诊断有重要意义。螺旋CT有很高的密度分辨率，不但能够显示积气部位，还能够鉴别积气的原因和发现其他合并症，对指导临床治疗有重要的应用价值。本文回顾性分析2004年7月至2007年8月间经X线胸片和螺旋CT确诊的50例纵隔气肿影像表现，旨在提高其诊断水平。 毕业论文。

; 自发性纵隔气肿20例，其中男15例，女5例，年龄19岁~80岁，平均年龄30.5岁。8例有剧烈运动、咳嗽等诱发因素，4例合并慢性肺部疾病史。临床主要表现为胸骨后疼痛、胸闷和呼吸困难。 毕业论文。
; 急性外伤性纵隔气肿27例，其中男22 例，女5例，年龄19岁～62岁，平均年龄39.5岁。车祸伤16例，高处坠落伤4例，重物压伤3例,刀刺伤4例。临床主要表现为不同程度胸痛、胸闷、气促、休克，4例因合并症死亡。 毕业论文。
; 食管破裂致纵隔气肿2例， 1例为洗胃时胃管刺破食道所致，行急诊手术治疗，1例为鸡骨头刺破食管所致，转上级医院进一步治疗。 ; 医源性纵隔气肿1例，男，54岁，行胸椎骨折内固定手术所致上纵隔积气，经保守治疗后好转。 毕业论文。
; 50例中，住院患者40例，门诊患者10例，行X线胸片检查36例，其中28例仅摄正位胸片，8例同时摄正侧位胸片。行螺旋CT检查41例。同时行X线胸片和螺旋CT检查27例。

; X线平片检查诊断明确纵隔气肿26例，怀疑诊断4例，漏诊6例。胸部正位片表现为纵隔旁线条样透亮影10例，纵隔内多发不规则条片状影16例，侧位胸片显示更清晰。合并颈、胸部皮下气肿18例，气胸16例，肋骨骨折24例，肩胛骨骨折4例，四肢骨折9例。 毕业论文。
; 螺旋CT检查全部诊断明确41例，其中前纵隔气肿5 例，中纵隔气肿7 例，后纵隔气肿3例，弥漫性纵隔气肿26例。合并颈、胸部皮下气肿34例，肺挫伤30例，胸腔积液28例，气胸34例，肋骨骨折17例，肺炎3例，食管破裂2例，肺气肿、肺大泡6例，脑外伤出血20例，内脏器官破裂6例，膈下游离气体2例（1例手术证实为外伤性十二指肠破裂，1例合并心包出血死亡）。 毕业论文。
