Effect of allograft kidney tissue mass implantation on EPO of rats with chronic renal failure。

ABSTRACT: Objective To study the effect of kidney tissue mass implantation into the capsule of rat kidney on EPO of rats with chronic renal failure. Methods After the models of chronic renal failue (CRF) induced by adriamyucin were set up in Wistar rats, the kidney tissue mass (1mm3) of newborn Wistar rats were located under the capsule of kidney of the models. During the process of treatment, EPO level in the serum was tested by EPO ELISA kit. Renal pathological changes were observed, and the expression of EPO in transplantation tissue was examined by Immunohistochemical method. Results ① The levels of EPO (1.160±0.324mU/mL) in the control group were lower than those of the other groups, with the difference being significant (P0.05). ② The grafts survived and increased in size from 1mm3 to 4mm3 in 60 days, around which there were plenty of newborn blood vessels. Histological examination showed that the grafts had normal structure of glomerule and uriniferous tubules. ③ Immunohistochemical study demonstrated EPO positive grains were located in the region of uriniferous tubules and there were more positive grains in renal tissue of the graft than in that of CRF (P0.05). Conclusion Allograft transplantation of kidney tissue mass can ameliorate renal anemia caused by chronic renal failure and might be a new therapeutic means to treat CRFinduced anemia.

摘要:目的 研究鼠婴组织移植于肾包膜下对促红细胞生成素调节作用,为临床应用治疗肾性贫血提供理论依据。方法 以Wistar雄性大鼠建立慢性肾功能衰竭动物模型为受体,将鼠婴组织块多点植入受体肾包膜下。治疗期间用EPO酶联免疫(ELISA)试剂盒测定血清EPO水平;观察肾组织病理改变,用免疫组化方法检测EPO在移植物中的表达。结果 ①60d时D组血清促红细胞生成素(1.768±0.140)mu/mL高于B组(1.160±0.324)mu/mL(P0.01),与C组(2.329±0.125)mu/mL也有差异 (P0.05)。②移植后60d,移植物的体积由1mm3增至3—4mm3大小,表面血管网丰富,光镜下见肾小球、肾小管结构正常;③移植物EPO免疫组化,发现移植物EPO着色颗粒主要分布于肾皮质的肾小管,明显多于病例对照组大鼠组织(P0.05)。结论 通过对移植物的形态观察和功能测定,证明肾组织移植是一种有效的治疗手段,有可能为慢性性贫血提供一种新的途径。

慢性肾衰所引起的贫血(肾性贫血)常常难以改善。其临床重要性越来越受到人们的关注。其主要原因为促红细胞生成素(EPO)分泌不足。目前应用EPO存在价格高、副作用等问题,因此肾性贫血的治疗日益引起人们的重视。此研究采用同种鼠婴组织为供体,慢性肾衰雄性大鼠为受体,进行同种肾组织移植的实验。现报告如下。

1 材料与方法。

1.1 实验动物 Wistar健康雄性大白鼠,体重(200±20)g,共80只,Wistar鼠婴20只,出生3—4d,雌雄不分,供移植用(由山西医科大学实验动物中心提供,按清洁级标准饲养)。

1.2 主要试剂 重组人类促红细胞生成素(2000u/mL,山东阿华医药公司,批号99435;大鼠用量按文献方法折算)。EPO免疫组化试剂(天津TBD生物工程有限责任公司)。F12培养基(Sigma公司)。肌酐和尿素氮试剂盒(北京化康泰公司)。

1.3 主要仪器 LD42A台式离心机(北京医用离心机厂)、KJB快速旋涡混匀器(江苏省姜堰市健康医疗器具有限公司)、DK(cu600)电热恒温水温箱(上海医用恒温设备厂)、ELX800酶标仪(美国BIOTEK公司)、UV760紫外可见分光光度仪(上海分析仪器总厂)、OLYMPUS图象分析仪(上海分析仪器总厂)。

1.4 实验动物分组与给药 A组:正常对照组(n=20)。B组:病例对照组(n=20)。C组:rHuEPO治疗组30u/kg,腹腔注射,1周3次,连续6周(n=20)。D组:肾组织移植治疗组(n=20)。

1.5 动物模型制备 经鼠尾静脉注射盐酸阿霉素6.5mg/kg,三周后行右肾切除,制成稳定的慢性肾功能衰竭大鼠(CRF)模型。

1.6 移植方法 用引颈法处死鼠婴,然后把整个小鼠浸入盛有75%(体积分数)酒精的烧杯中;2—3s后取出拭去酒精放入碟皿中,继之在动物腹部用尖刀纵行切开皮肤取婴肾;置入盛有Hanks液的消毒青霉素小瓶中,反复剪切组织至1mm3大小为止(成糊状);静置使组织下沉,弃上清液,加入37℃的F12培养基待用[1]。受鼠用10%(体积分数)水合氯醛4mL/kg体重麻醉后,俯卧并将四肢固定,剪去背部毛发;手术野消毒后切开皮肤、肌肉至显露肾脏,于左侧肾脏腹面横形切口长约3mm;之后用睫状体分离器分离肾包膜使之成囊袋,取10—15块肾组织置于左肾包膜下,小块相互距离为0.5cm,之后用尼龙线缝合包膜,逐层缝合切口。移植后不用免疫抑制剂[13]。

1.7 标本采集 分别于实验0、15、30、45、60d经大鼠内眦静脉采血做指标检测,60d时处死动物摘除肾脏,观察大体外观,沿肾门纵切,置10g/L甲醛固定,送病检。
